Nurse (LPN) to provide compassionate, one-on-one care for patients
in their homes. This position is ideal for LPNs with strong
clinical skills, experience in direct patient care, and a passion

Supportive Management Team to ensure your success Responsibilities
Assess and monitor patients’ conditions, providing direct nursing
care Administer medications and treatments as prescribed Educate
and support patients and their families regarding care plans and
medical conditions Collaborate with physicians and the broader
healthcare team Maintain accurate and timely documentation of
patient care Communicate effectively with the patient’s primary
healthcare providers Requirements Current and active LPN license in
the state of Arizona Minimum 1 year of experience as an LPN
preferred but not required Strong communication and interpersonal
skills Ability to work independently and manage time efficiently
Demonstrated commitment to compassionate, patient-centered care
Valid driver’s license and reliable transportation Bilingual is a
plus! Schedule Both full-time and part-time opportunities are
available with flexible scheduling , including day, evening, and
weekend shifts. Shifts typically range from 6 to 12 hours , based
