Position Type: Locum tenens (contract healthcare job) Schedule: Full-time, Monday – Friday (no weekends, no on-call) Shifts per Week: 5 Shift Duration: 8-hour days Assignment Length: 26 weeks Start Date: December 8, 2025 Estimated Pay Range: $120,000 – $140,000 per year Setting: Hybrid – 3-4 days in-home patient visits, 1-2 days remote (admin/telehealth) Advanced Practice Provider Qualifications Bilingual proficiency required (Spanish/English preferred) Minimum 2 years of experience as a Nurse Practitioner or Physician Assistant Background in home health, primary care, or internal medicine preferred Experience managing care plans is a plus Not suitable for urgent care, hospitalist, surgical, or pediatric backgrounds Ability to travel up to 1 hour for patient visits Current and unrestricted Arizona Nurse Practitioner or Physician Assistant license Healthcare Job Responsibilities Conduct initial and follow-up assessments for patients with chronic kidney disease and end-stage renal disease Perform comprehensive physical exams, including vitals and head-to-toe assessments Educate patients on disease processes and medication management Coordinate hospital transitions and manage patient care plans Visit 4-5 patients per day (occasionally up to 8) Document patient encounters using Athena and Salesforce EMR platforms Participate in POD rounding to discuss high-risk patients and care updates Utilize AI-powered scheduling tools to optimize routes and patient visits Engage in remote administrative work, including charting and telehealth visits Why Choose Sierra Vista, Arizona?
