Huachuca Fort Huachuca is a United States Army installation under
the command of the United States Army Installation Management
Command. It is located in Sierra Vista in Cochise County, in
southeast Arizona, about 15 miles north of the border with Mexico.
Beginning in 1913, for 20 years the fort was the base for the
"Buffalo Soldiers ", the 10th Cavalry Regiment. During the buildup
of World War II, the fort had quarters for more than 25,000 men and
hundreds of WACs. In 2010, Fort Huachuca has a population of
roughly 6,500 active duty soldiers, 7,400 family members and 5,000
civilians. Fort Huachuca can have well over 18,000 people on post
during the peak hours of 0700 and 1600, M-F, making it one of the
busiest transient installations. Requirements Examines active duty,
family members and other authorized beneficiaries to determine the
presence and extent of diseases and conditions. Administers
appropriate care and or referral of cases requiring hospitalization
or subspecialty care. Shall become the primary care manager (PCM)
for a panel of patients. As a PCM, the Physician Will be the
primary person responsible for the management of the health and
wellness of his/her assigned patients. Shall participate, as
appropriate, in departmental Quality Assurance (QA) programs and
adhere to all safety regulations at all times. The HCP provides
technical advice and professional opinions upon request from the
MTF Commander, DC, and/or Clinic Chief, or other assigned
representative. Shall attend and participate in meetings, patient
care reports, patient care conferences, team conferences, case
conferences, professional staff conferences and trainings, as well
as, other appropriate professional activities to the extent that
such attendance and participation is relative to assigned cases
and/or performance of contract services as determined the MTF
Commander, the DC, the Chief of the PCMH Clinic or other assigned
representative. Become familiar with the hospital formulary and be
authorized to prescribe pharmaceuticals shall do so according to
the availability of drugs listed therein. The pharmacy service will
provide instructions to all prescribing practitioners on
substitutability of generic drugs for prescribed drugs. Drugs not
on the formulary or approved by the Pharmacy and Therapeutics
Committee will not be utilized unless a new drug request is
submitted and approved. The HCP shall use computer order entry to
order prescriptions, or use prescription forms DD 1289 in
accordance with local policy. Complete all documentation of care
provided in the MTF computer programs for which training was/is
provided. Ensure all patient encounters are closed within 72 hours
of encounter. Qualifications Must have graduated from an accredited
Doctor of Medicine (MD) or Doctor of Osteopathy (DO) program to
include an Internal Medicine residency program. Must be Board
Certified in Family Medicine by the American Board of Family
Medicine (ABFM) or American Osteopathic Board of Family Medicine
(AOBFM). Must have a minimum of two (2) years of full-time
experience within the last five (5) years as a practicing Family
Medicine Physician prior to commencing services. Possess and
maintain a valid, unrestricted license to practice medicine in one
of the 50 United States, District of Columbia, Commonwealth of
Puerto Rico, Guam or U.S. Virgin Islands. Possess and maintain at
all times a current Drug Enforcement Administration (DEA)
registration. Possess and maintain certification in Basic Life
Support (BLS) as approved by the American Heart Association (AHA).
